The first thing that comes to my mind with that title is R.E.M.'s album that included one of my favorite R.E.M. songs, the appropriately emo titled Everybody Hurts. I don't think that reference is the point of the episode and if anyone else has a reference for the title of the episode I really need to hear it because I got nothing.
I found the episode pretty confusing. It seems like alot of stuff is being introduced, questions being asked but nothing is getting resolved or answered.

-- Trust seems to be a big theme, one that pervades not only the episode but also the season. Who can you trust? As more Terminators are introduced (especially the shapeshifting T-1000) we're forced to consider who and what we're dealing with at any given moment.

That house is SO Sarah's apology to John. Let's see how long it takes for them to destroy it.
-- Messages from the future in a body bag, with a whole in the chest. So, what's changed because obviously something from Samson and Delilah has drastically affected the timeline.
-- Carrots and Apples. Bitch from the future!!!
So nothing happens just to happen and no one is just introduced for the sake of new love interest. Everything in this show is about plot, so my question is WHO IS SHE? There was that random chicken from S1 with the abusive parents and TPTB did NOTHING with that plot line to date. If Riley is the fall out from that, then Boo Hiss! She's not interesting or cool. Could they have at least made her interesting or quirky in a not annoying way?
-- Pissy teenage hissy fit. Guess cutting off the emo hair didn't get rid of the emo angst. We're continually reminded that John is a KID. I'm desperate to find out when and how he makes the transition to the man that will lead armies. Does that transition necessitate Sarah's death because to date he whines about everything and expects her to fix what's wrong. John's dependence on her doesn't give me any hope for the future of mankind because he acts like a spoiled teenager so often, never taking into consideration the sacrifices Sarah makes and has made on his behalf.
-- How did Sarah Connor get cancer? Oh, gee ... lemme think. I found that closing sequence confusing and curious. That Sarah would risk radiation poisoning to help Cameron who is a Terminator that can handle itself, a thing she does not trust. I'm kinda wondering if she's beginning to attach to Cameron because John is distancing himself from her. I can see Sarah floundering under the strain of John's response and her idea of her responsibility to him.
-- Sarah uses her femininity as a way to gain information. She enacts her gender in in very specific circumstances, otherwise we can read her as either masculine or even gender neutral.
